The NHS in England is now using an electronic record called the Summary Care Record (SCR), which is being used to support patient care. The summary care record details key information held in your GP record including medication you are taking, any allergies you may have, which if required and with your authority, can be shared with other health care providers.
All patients have a choice as to whether or not an SCR is created for them. If you are registered with this practice and choose to have an SCR , then you do not need to do anything. However, if you are registered with this practice and do not wish to have an SCR created, then you will need to opt out.
